REZUM – Does it Work for Everyone?

REZUM is an effective procedure that offers numerous advantages to many men with an enlarged prostate. It works for many men but not for all men. The success of treatment depends on patient characteristics including the severity of symptoms, size of the prostate, and severity of blockage caused by an enlarged prostate.

What Can Be Done If REZUM for BPH Does Not Work?

While many men experience an improvement of urinary symptoms after the REZUM procedure, some men experience no improvement or insufficient benefit from the REZUM procedure. Other men who obtain significant benefit from the REZUM procedure may subsequently experience deterioration of urinary problems caused by BPH sometimes as soon as 1-2 years after the procedure or later, 3-5 years after the procedure. Determining the Cause of REZUM Treatment Failure

It is important to determine the cause of urinary symptom recurrence or failure after the REZUM procedure to provide adequate benefit. Sometimes the reason may be simply inadequate treatment but other times symptoms may be caused by a different problem such as the overactive bladder, bladder stones, bladder cancer, or urethral stricture.

Treatment Options if REZUM Procedure Does Not Work.

At New York Urology Specialists we have experience treating men after failure or inadequate benefit of the REZUM procedure. Fortunately, most treatment options used for the treatment of enlarged prostate remain available. The choice of treatment depends on full evaluation, the severity of the blockage, and patient preferences.

Repeat the REZUM Procedure?

While repeating REZUM is an option, typically repeating the same procedure will not lead to dramatically improved results. If REZUM failed to deliver once, it is typically a sign that it is unlikely to perform dramatically better if repeated.

Minimally Invasive Options for BPH

Among minimally invasive options, Urolift for BPH is a procedure that has shown to be effective for men with an enlarged prostate. Urolift can be performed for many but not all men who did not benefit from REZUM. The advantage of Urolift in this situation is that it can be performed in the office under local anesthesia; typically there are no sexual side effects and recovery is fast. Men can expect to experience moderate improvement in symptoms after the Urolift procedure. The procedure takes about 15 minutes to perform. Prostate enucleation is an effective treatment option for men regardless of prostate size. HOLEP is a prostate enucleation procedure performed with a holmium laser. Prostatic artery embolization can also be performed and may be effective for some men. Aquablation for BPH with AquaBeam is another new option for some men with an enlarged prostate who fail treatment with Rezum.

Medications for BPH

Medications such as Rapaflo, Uroxatral, and Flomax can be tried and may often improve symptoms if not tried prior to REZUM. Finasteride can be tried but it typically takes 3-6 months to see maximum benefit. Cialis 5 mg daily for BPH is another option that may relieve mild-to-moderate symptoms of BPH after REZUM for BPH procedure.

Surgical Treatment Options for Enlarged Prostate if REZUM procedure does not help

Surgical treatment options typically provide the most relief of symptoms. While open or robotic suprapubic prostatectomy typically provides the best improvement of symptoms, it is a more invasive procedure with a longer recovery. For most patients, an equally effective treatment option is a relatively new procedure called laser enucleation of the prostate. Other treatment options after REZUM include traditional TURP, bipolar TURP, Plasma button TURP, various laser ablation procedures such as Greenlight Laser, HOLAP/HOLEP – Holmium laser procedures). The actual choice of the procedure after REZUM is determined by the size and anatomy of the prostate as well as patient preferences. At many hospitals only one or two technologies are available and the patient’s choice is limited by the procedure/treatment.
