Does a woman have a prostate?

In women, the prostate gland, although generally smaller than the male prostate, also surrounds the urethra, close to the urethral opening. The great sensitivity comes not from what is on the upper wall of the vagina but from glands and ducts behind the vaginal wall.

To the editors:

1 Hines TM The G-spot: a modern gynecologic myth. I appreciated Hines' very interesting article “The G-spot: A modern gynecologic myth” in the August 2001 issue of the Journal. 2 Gräfenberg E. The role of the urethra in female orgasm. Part of the trouble with this article and the entire discussion is the lack of agreement on definitions. The G-spot, or Gräfenberg spot, does not refer to “a small but allegedly highly sensitive area on the anterior wall of the human vagina, about a third of the way up from the vaginal opening.” Instead, it refers to the “area” or “zone” on the upper wall of the vagina through which the prostate (also known as Skene's glands and ducts) can be accessed. In women, the prostate gland, although generally smaller than the male prostate, also surrounds the urethra, close to the urethral opening. The great sensitivity comes not from what is on the upper wall of the vagina but from glands and ducts behind the vaginal wall.

Conway-Welch C The role of the Grafenberg spot and female ejaculation in the female orgasmic response: an empirical analysis. The biggest problem I have with the Hines article is how his review of the relevant literature that support the existence of a female prostate gland as the so-called G-spot misinterprets some research and either ignores or is unaware of other relevant findings. He even promises to discuss that the female prostate gland is indeed the G-spotthat he finds so hard to locate, but he never really does discuss this.

Whipple B Update on the female prostate and the phenomenon of female ejaculation. There have been numerous pathologic studies that in some way support the conclusion that what has been called Skene's or paraurethral ducts and glands are, in fact, not a vestigial homolog of the male prostate but, instead, a “small, functional organ that produces female prostatic secretion and possesses cells with neuroendocrine function, comparable to the male prostate.” The Hines article clearly demonstrates that the term “spot” is not a useful metaphor to describe the anatomic basis of the female erogenous experience of stimulation of the upper vaginal wall. It is only contributing to the confusion. A more accurate and scientifically based concept, such as the female prostate, should make it easier for everyone to understand the issues involved and better serve women's health needs. The G-spot is an area of enormous importance to millions of women and their partners in terms of experiencing more pleasure and intimacy from their sexuality.

What is a woman's most fertile time?

Ovulation happens about 14 days before your period starts. If your average menstrual cycle is 28 days, you ovulate around day 14, and your most fertile days are days 12, 13 and 14. If your average menstrual cycle is 35 days ovulation happens around day 21 and your most fertile days are days 19,20 and 21.

Facts about timing

Ovulation is when a mature egg is released from the ovary. The egg then moves down the fallopian tube where it can be fertilised. If sperm are in the fallopian tube when the egg is released, there is a good chance that the egg will be fertilised, creating an embryo, which can grow into a baby. Pregnancy is technically only possible if you have sex during the five days before ovulation or on the day of ovulation. But the most fertile days are the three days leading up to and including ovulation. Having sex during this time gives you the best chance of getting pregnant. By 12-24 hours after ovulation, a woman is no longer able to get pregnant during that menstrual cycle because the egg is no longer in the fallopian tube. There’s almost no chance of getting pregnant if you have sex before or after the fertile window (but if you’re not trying to get pregnant, don’t rely on this – contraception is your best option!).
